FAQ: ALTCS Paid Family Caregivers

Can spouses be paid family caregivers?

Yes. All cases need approval, but ALTCS will cover caregiving situations where the caregiver is the spouse.

Yes. All cases need approval, but ALTCS will cover caregiving situations where the caregiver is a parent.

When is the pay tax-free?

Under IRS Notice 2014-7, caregivers who live in the same home as the person they are caring for may not have to pay federal income tax on the wages they receive through My House In-Home Care. This "Difficulty of Care" exclusion helps families keep more of their compensation to support their household. The exemption applies regardless of the relationship: caregiver, friend, spouse, child, sibling, grandchild, etc. Other criteria may apply. Consult with a tax professional to see how this applies to your specific situation.

Do I pay for the training?

This can vary between home care agencies. With My House In-Home Care, we provide all the needed training free of charge.

What if the paid family caregiver gets sick or needs a break?

The level of support can vary between agencies. My House sends professional personal care aides to give paid family caregivers a break whenever that is needed. Medicaid/ALTCS pays up to 100% for this respite care.

Must the caregiver be family?

No. The caregiver can be anyone the family chooses, as long as they are physically able to do the work, legal to work, and have a clear criminal background check. The person could be a friend or even someone you recruited or interviewed. People often choose a family member for trust and familiarity, so the benefit is often referred to as “paid family caregivers,” as a shorthand.

Must I choose my own caregiver?

While many clients prefer this, it is not required. My House In-Home Care has a staff of well-trained, professional direct care workers. ALTCS will pay for us to recruit, train, and schedule direct care workers for your home.

When can I start getting paid?

This varies between agencies. My House In-Home Care is unusually fast. Caregivers chosen by the family/client can start working for pay in 24 to 48 hours! If a family chooses their own caregiver, My House In-Home Care can complete its employee onboarding in 24 to 48 hours in most cases. Training takes time, but caregivers chosen by a client can start working for pay as soon as they are hired.

Will the paid family caregiver be an employee?

Yes. The caregiver chosen by the family or client will be hired by My House In-Home Care. We will provide all the usual services of an employer, such as training, payroll, deducting taxes, workers' compensation insurance in case you are injured on the job, background checks, and backup services for when the usual caregiver is unavailable.

How do I get started?

We have two offices — one in Mesa serving the East Valley and greater Phoenix area, and one in Tucson serving Pima County. My House has been providing home care services in Arizona since 2006 — nearly two decades of serving families in Mesa, Tucson, and the surrounding communities. That history means we understand the local Medicaid and ALTCS landscape deeply, have long-standing relationships with case managers, and have built a team and culture that families trust.

My House is 100% locally owned and operated — not a national franchise. Every decision about how we serve our clients is made right here in Arizona, by people who live and work in the same communities as you do. Does My House serve both English- and Spanish-speaking families?

Yes — se habla español. My House serves a large Spanish-speaking population across both our Mesa and Tucson offices, with bilingual staff and caregivers available. We also work hard to accommodate other languages — Arizona is one of the most diverse states in the country, and we believe language should never be a barrier to care. If your family speaks a language other than English or Spanish, please call us and we will do our best to connect you with someone who can help.

What home care services does My House provide?

My House provides a comprehensive range of in-home services for seniors, adults with disabilities, and individuals recovering from surgery or illness.

Does My House provide 24-hour or overnight care?

Yes. My House provides 24-hour and overnight care for clients who require around-the-clock support. This is available for both ALTCS members whose care plan authorizes sufficient hours and for private-pay clients. If you are not sure whether 24-hour care is covered under your loved one's ALTCS plan, call us and we will review the authorization details with you.

Do you provide care for people with Alzheimer's disease or dementia?

Yes, and this is one of our areas of focus. My House caregivers receive specialized dementia and Alzheimer's care training as part of the Level 2 Aging and Physical Disabilities Direct Care Worker curriculum. We understand the unique challenges that memory loss, wandering, confusion, and behavioral changes create — both for the person living with dementia and for their family. We work to match dementia clients with experienced caregivers who bring patience, consistency, and genuine compassion to every visit.

Can My House help with care after surgery or a hospital stay?

Yes. Post-operative and post-hospitalization home care is one of the most common reasons families call us. After a surgery, hospitalization, or stint in rehabilitation, returning home safely requires real support — help with bathing and dressing, meal preparation, medication reminders, mobility assistance, and monitoring for changes in condition.

Does My House provide respite care for family caregivers?

Yes. Family caregivers are the backbone of the long-term care system, and they deserve time to rest, recharge, and attend to their own health and obligations. My House provides respite care — temporary relief care — so that family caregivers can take a break without worrying that their loved one is not being looked after. Respite care can be arranged for a few hours, a full day, or longer stretches depending on need and authorization. ALTCS members may have respite care included in their authorized care plan.

How are My House caregivers screened and trained?

Every My House Direct Care Worker goes through a thorough vetting and training process before they ever set foot in a client's home.

  • Negative TB test on file

We also conduct ongoing quality checks including supervisory visits at 5 days, 30 days, and quarterly thereafter, plus random EVV verification and regular client satisfaction calls.

How does My House handle scheduling and what happens if a caregiver cannot make it?

We build consistent schedules, so clients see familiar faces and build trust with their caregiver. If a scheduled caregiver is unavailable due to illness or an emergency, our coordinators work to find a replacement as quickly as possible — based on the contingency plan we put in place at the start of services.

Every client has a written Contingency Plan that documents how quickly backup care is needed based on the client's functional level. Options range from coverage within 2 hours to the next scheduled visit, depending on the client's preference and medical needs. Our on-call line is available after hours at (480) 773-5922 for Mesa and (520) 331-3051 for Tucson.

How do I know care visits are happening as scheduled?

My House uses Electronic Visit Verification (EVV), a federally required system that electronically records the start time, end time, location, and tasks completed at every care visit. Caregivers use the WellSky mobile app or telephony to clock in and out at the client's location.

This data is monitored daily by our office staff and is accessible to AHCCCS and your MCO case manager. You never have to wonder whether care was provided — it is verified, time-stamped, and on record. My House supervisors also conduct regular in-person supervisory visits and follow up with clients by phone to confirm satisfaction with services.

What should I do if I have a concern about the care being provided?

My House takes client and family concerns seriously and responds promptly. Our grievance process works like this:

  1. Talk directly with your caregiver if you are comfortable doing so. Many concerns can be resolved immediately.

  3. If the issue is unresolved, ask to speak with our Office Manager or CEO.
  4. You may also contact your MCO case manager at any time — they are your independent advocate and have authority to require corrective action from any provider.

All concerns are documented, investigated, and followed up on in writing. No client or family member should ever feel they have nowhere to turn.
